Hold your finger on the app until it jiggles. Tap ‘X’ in the corner of the first prompt, and confirm deletion for pop-up requesting confirmation to delete as well.

You cannot sign out or delete an account without knowing the password. That’s basic account security, so you will have to reset the password and recover it first before doing anything else.
